Social Work

Navigating Life's Hardest Moments Together

Social work support that connects people to real help and builds the capacity to manage what comes next. At Delton Health Care, our social workers help individuals and families work through the practical challenges, emotional weight, and complex situations that can make life feel genuinely unmanageable. We connect clients to community resources, assist with NDIS and aged care navigation, provide counselling, and offer the kind of consistent, human support that actually makes a difference when things are hard.

We are not a referral service. We are a team of skilled clinicians who sit with people in difficult circumstances and help them find a clearer path forward.

We support NDIS participants, older Australians, people experiencing mental health challenges, and families navigating significant life changes.

  • NDIS plan navigation and service connection support
  • Crisis intervention and practical problem-solving
  • Individual and family counselling
  • Carer support and family consultation
  • Advocacy within systems and with service providers
  • NDIS and aged care funding accepted

Why Choose Delton Healthcare for Social Work?

Practical Help With Complex Systems

Navigating the NDIS, aged care funding, housing, or community services can be exhausting. Our social workers know these systems well and help clients access the support they are entitled to without the confusion and delays.

Counselling and Emotional Support

Beyond practical coordination, our social workers provide direct therapeutic support for people working through grief, trauma, family breakdown, carer stress, or significant life changes.

Advocacy That Puts Clients First

Sometimes people need someone to speak up for them. Our social workers advocate within systems and with service providers to make sure our clients' rights and needs are heard and respected.

Holistic, Trauma-Informed Practice

We apply a trauma-informed lens to every client relationship. This means understanding behaviour in context, building trust over time, and working at the client's pace rather than the system's.

Our Social Work Services

NDIS Navigation and Service Coordination

The NDIS is one of the most significant support systems available to Australians living with disability, but accessing it well takes knowledge, time, and persistence. Many participants are not getting the full value of their plans simply because they do not know what they are entitled to or how to connect with the right providers.

Our social workers help clients understand their plans, identify gaps in their current supports, connect with appropriate services, and navigate the review and appeal processes when things are not going as they should.

  • NDIS plan review preparation and goal setting support
  • Service agreement support and provider connection
  • Assistance accessing and coordinating community services
  • Support through NDIS plan reviews and appeals
  • Coordination with the broader Delton allied health team

Social work counselling is different from psychology in its scope, but it addresses many of the same human experiences. Grief. Carer stress. Family conflict. The emotional weight of a new disability diagnosis. The challenges that come with ageing. The isolation that can build when life becomes harder to manage.

Our social workers provide a consistent, supportive relationship where clients feel genuinely heard and where the conversation can go wherever it needs to go.

  • Grief and bereavement counselling
  • Carer stress and burnout support
  • Family relationship challenges and communication support
  • Adjustment to disability or chronic illness
  • Mental health and wellbeing support for NDIS participants
  • Crisis support and safety planning

Families facing complex challenges, whether related to disability, mental health, domestic difficulties, or significant change, often need support that addresses the whole family unit, not just the individual. Our social workers work with families to strengthen relationships, improve communication, and build the capacity to manage together.

  • Family assessment and support planning
  • Parenting support for families of children with disability
  • School and community connection support
  • Child wellbeing and safety planning
  • Coordination with schools, health services, and community organisations

When someone is in a crisis, whether that is a housing emergency, a family breakdown, a mental health episode, or the sudden loss of a support arrangement, they need someone who can respond quickly, think clearly, and help them take the next step.

Our social workers are experienced in crisis situations and bring both practical problem-solving skills and a calm, grounded presence to some of the hardest moments our clients face.

  • Safety assessment and crisis support planning
  • Emergency housing and financial assistance referrals
  • Support for people leaving unsafe situations
  • Coordination with emergency services and community organisations
  • Follow-up support and stabilisation after a crisis

More Than a Referral. Real Support.

Social work is about more than connecting people to services. It is about helping them feel less alone in difficult circumstances and more capable of managing what comes next.

Our social workers engage deeply with the people they support, building relationships that create the safety and trust needed for meaningful change to happen.
